Instructions for Filling the Online Registration Form for Doctors
Please read the instructions carefully before you submit the application form to
avoid any inconvenience.
The Form is bilingual. You are required to fill some of the details in English as
well as in Hindi. Your name should be filled in the application form as it appears
in your SSC / Hr. Sec./ Diploma / Degree Certificate.
Check the spelling of your name and father's name filled by you before submitting
the application form.
Address & Name should be written in capital letter.
Please enter the name of the authority (University / Board / Council) in the same
manner as it appears in the Degree Certificate.
Please ensure that copy of all documents has been attached with the application.
Please bring all the
original certificates for verification in Homoeopathic Medicine Board, Dehradun.
Please bring the two character certificate given by two different Gazetted Officer
working in Uttarakhand Govt. seperatly by bring it in person & it should not be more than 6 months old.
All the certificates should be upload online & self attested.
To avoid inordinate delay the Council shall return all incomplete applications.
Kindly ensure that the copies are clear and the name, date of birth,
qualification, signature of the issuing authority are clearly apparent. No overwriting
on the Xerox copy is permissible.
The application must ensure that the name indicated on the application form should
be the same as given on the degree as well as the date of birth certificate(date of birth in christian era). In case of any
discrepancy form is likely to be rejected.
Enclosures for Registration who has Passed Degree / Diploma Examination
from Uttarakhand:-
A photo copy of High School, Intermediate marks sheet
& certificates should be self attested.
Two character certificate original obtained by two different Gazetted officer
working in Uttarakhand Govt. service with seal & date, known time,contact no. & full address.
A photo copy of Internship completion certificate (Obtained by the Principal from the college
candidate has passed Degree/ Diploma examination) should be self attested.
Two photo copy of BHMS/DHMS/Others 1st year, 2nd year, 3rd year and Final year marks sheets should be self attested.
Two photo copy of BHMS/DHMS Degree/Diploma certificate should be self attested.
Two colors photographs affix on plain paper, should be self attested.
One 10"x12" envelope having registered ticket.
Two colors photographs none attested.
A photo copy of Domicile certificate of Uttarakhand or address proof, self attested.
Payment of registration fee would be online mode only.
(Please remember that it is a legal document & has to be furnished correct in all
respect, and self attested )
Enclosures for registration who has passed BHMS Degree examination from other than Uttarakhand
NOC from concern state where candidate has done permanent registration.
A photo copy of High School, Intermediate marks sheets
& certificates self attested.
Two character certificate original obtained by two different Gazetted officer
working in Uttarakhand Govt. service with seal & date, known time & full address.
A photo copy of BHMS 1st,2nd,3rd year,final year marks sheets duly attested by self.
A photo copy of BHMS Degree/ Provisional degree should be self attested.
Two colors photographs affix on plain paper should be self attested.
Two colors photographs non attested.
Payment of registration fee would be online mode only.

आवेदन पत्र को भरते समय जो भी अभिलेख संलग्न किये जाने का उल्लेख है, यदि वह अभिलेख ऑनलाइन संलग्न नही किया गया है, तो आपके आवेदन पत्र को निरस्त कर दिया जायेगा तथा आप द्वारा भेजा गया शुल्क बोर्ड द्वारा वापस नही किया जायेगा।
शपथ पत्र रू0 10/- के नाॅन ज्यूडिशियल स्टाम्प पेपर एवं यदि आप भारत वर्ष के किसी अन्य प्रदेश के होमियोपैथिक बोर्ड / कौउसिंल से पंजीकृत है अथवा नही, इस आशय का भी शपथ-पत्र रू० 10/- के नॉन-जुडिशल स्टाम्प पेपर पर (मूल रूप से) पंजीकरण फार्म के साथ संलग्न करना अनिवार्य है ।
